Terry is currently enrolled in the 9th grade at South Floyd High School where he is experiencing academic difficulty. Terry is a 14 year old, white male of seemingly average height and weight with sandy brown hair and green eyes. He appeared appropriately dressed and exhibited adequate hygiene. According to vision and hearing screenings conducted at the school, no difficulties were noted for vision, and speech/language/communication is average. According to school records, Terry’s attendance was indicated as very good. Terry lives with his grandmother and his father. According to Terry’s teachers he has difficultly keeping up with his peers academically. Terry’s teachers also indicate that he has difficultly getting along with others and has a problem with following directions, as well as difficulty completing The day of testing there were no observable physical disabilities. During the evaluation, Terry used his right hand and his behavior was marked as normal, with his body movements indicating no gross or fine motor impairments. Terry appeared secure
