However, as an advocate for the school of psychoanalysis treatment, Dr. Cawley (Ben Kingsley) and Dr. Sheehan (Mark Ruffalo) make every effort to persuade their superiors to try one more time to cure Laeddis. In that case, a delicate and suspenseful role play starts. In his scenario of hallucination, Laeddis set himself in it as a second persona, Teddy Daniels (an anagram of Andrew Laeddis), who is U.S. marshal. Apparently, he and his new partner, Chuck Aule (Dr. Sheehan) are undertaking the investigation for the disappearance of a dangerous patient, Rachael Solando (an anagram of Dolores Chanal) on the Shutter Island. However, in fact, Teddy secretly searches for "Andrew Laeddis", an arsonist who causes the fire that burns his wife to death. The investigation is not that smooth, for "Teddy" finds that everyone on the Island, including the doctors, nurses, and even the patients are withholding the truth from him. Suspicious of their attitude, "Teddy" associates his experience during the liberation of Dachau death camp – psychiatric experiments on living humans – and believes the same thing is also conducted on this island. One by one, "Teddy" investigates the ward A, B and C and finally the lighthouse. When he eventually gets to the lighthouse, he does not find the Nazi laboratory but simply Dr. Cawley sitting alone. It is the final confrontation. Face to face, eye to eye, Dr. Cawley breaks through Laeddis' heroic fantasy and enforces him to look straight at reality, his love, guilt, trauma and sin. The therapy measure of role play is a success. Laeddis eventually wakes up from the dream. Nevertheless, perhaps the past is too heavy a burden, perhaps life is too daunting to live; he at last consciously chooses to take the transorbital lobotomy rather than live soberly in the world. In the decision of "[living] as a monster or die as a good man", he chooses to "die" (Shutter).
