BioPsychosocial Assessment


            Michelle Valencia is a 22 year old female who is of Latino race and of Puerto Rican and Cuban ethnicity. Her religion is Pentecostal and her marital status is currently single. Her current employment status is working as a host at Olive Garden. Michelle is a Lesbian. Michelle is faced with living below the poverty line due to her being considered homeless, jumping from house to house for shelter. Her adopted parents are Betty Jackson and Charles Jackson. Michelle and her two siblings were adopted by Betty and Charles Jackson at a young age. Michelle received her G.E.D in the year of 2010. Michelle has a large amount of friends; she considers them to be somewhat like family. Michelle current disabilities consist of having Bipolar disorder, ODD (Oppositional Defiant disorder and ADD. When meeting Michelle her appearance was clean cut. She dressed for the hot weather, having worn a long maxi dress and sandals. Michelle speech was hesitant at first but she became comfortable after a couple of minutes. Her attitude was nervous but relaxed, cooperative and patient. In addition Michelle speaks three languages English, Cuban and Spanish. .
             Referral Source.
             The referring source is Stanley Isaac's Mental Health Center. The referring source states that Michelle current problems are being homeless, having the ability to cope with her disorders, depression issues and current family functioning. The client has been seeking treatment both voluntarily and involuntarily. She wants to get help but doesn't want to take the steps in order to change her current situation. Michelle does understand the reasons to why she was referred to Stanley Isaac's mental health center and why she chooses to receive help.
             Presenting Problem .
             Michelle is currently homeless because of her choosing to move out of her adopted parent home at 18 years old. Michelle is currently seeing a psychiatrist for her bipolar disorder at Katonah New York.
