Counseling Intake Assignment

 

The client said that she saw her parents taking cocaine at home as a child and that her parents would fight a lot. This occasionally resulted in domestic abuse from her father, with constant threats to leave from each parent. .
             Personal History.
             The client described herself as a recovering alcoholic working a long-term program of recovery. She mentioned that she worked as a model for 15 years and stated that she now feels she has a lack of purpose in life. The client described several areas of her life that she has had difficulties in, such as addiction and previous home-life with her parents and siblings. However, it appears that she is already self-actualising as a result of her time in support groups and treatment centres for her alcoholism. .
             Currently, the client believes that the main problem in her life that has the biggest impact on her is the lack of sex and intimacy in her marriage. The client said that she has always felt there was a lack of sex in her marriage, but that the worst time was after her baby was born when she suffered post-partum depression. At that time her alcoholism increased and since then (2009) there has been a major lack of intimacy and sex in the marriage.
             Medical and Mental Health History.
             The client started drinking and using substances at age 14. She has been clean and sober from drugs and alcohol now for over 3 years. She stated that this aspect of her life is now under control and her focus of improvement is in other areas, primarily her marriage and depression. The client mentioned, in regards to her health, that she smokes occasionally and has recently had a full body check up. She was given a clean bill of health for her physical health screening. The client was diagnosed with post-partum depression in 2008, shortly after the birth of her daughter, and was prescribed with a course of anti-depressants. She was on the medication for 5 years, during which time the client was drinking alcohol for the first four years.
