This patient denies any other addictive behaviors besides her substance abuse such as, spending, over-eating and gambling. The patient states that she also is aware of both her siblings being on antidepressant medications. She feels they all have depression due to their mother's death and childhood upbringing.
Previous Treatment History.
Patient states she has one prior treatment episode in 2010 when she checked herself into a thirty-day inpatient treatment program. She reports she successfully completing that program.
Substance Use History.
Patient has denied use of any alcohol or drugs prior to 2007 at the age of 30; when she met her partner who introduced her to methamphetamine. The patient's substance use has mainly been centered in methamphetamine, but admits to using alcohol on and off during this time. She began using one time a week to stay awake for her shift. She stated she thinks this lasted for about three months, before she knew it, she was using a gram almost daily, not eating or sleeping. In 2010, she felt her using was out of control and became severely depressed and did not want to die as her mother did. She was able to obtain some periods of abstinence ranging from three to six months. She eventually checked herself into an inpatient program.
Medical History – Patient states she has been having on-going dental problems, and lower back pain from falling off her bicycle.
Education and Work History.
Patient graduated from the University of California Berkeley in 2007 with her Master's Degree in Medical Science and obtained licensure with the State of California. The patient indicates that since obtaining her license in 2009, she has employment with the University of California, San Francisco Medical Center, as an emergency room Physician Assistant. She enjoys her work and is concerned with the outcome of her Medical Board hearing.
Legal/Financial Issues.
The patient states at this time she has no legal or financial issues that she is dealing with.
