Therapeutic Communication Analysis

It was a cool Friday morning on October 17th, 2014 at around 9:00 a. when I arrived at the patient's room. The patient's room was located on the Med-Surgical Unit, 512W, at Nazareth Hospital. The clients bed was located on the far side of the room next to the window. The client did not share the room with another patient. When I approached my assigned client I introduced myself. The client was sitting upright on the chair with his breakfast tray right in front of him. He wore a zipped-up fleece over his hospital gown. The client was receiving saline through his IV and also had a foley catheter.
The client was an elderly man who was 79 years of age from Philadelphia. He seemed to be enjoying the music that was playing on the television because he was nodding his head with the tune. When we made eye contact, I quickly smiled at him. I learned from the nurse his admitting diagnosis was urinary retention. Urinary retention is defined as the incomplete emptying of the bladder (Ackley et. al, 2012) Our conversation took place after he was done his breakfast. The first thing I observed in the clients room was the wrinkled and folded up linen on his bed. I politely asked the client if he wanted me to make his bed and provide him with new linen. We started our conversation when I brought over the fresh linen. I asked him, So what brought you to the hospital? He said he had problems urinating and has been struggling with his bowel movements for a couple months. This is the open-ended question technique to get to know the client. This technique is used to ask broad questions that lead or invite the client to explore thoughts or feelings (Kozier et. al, 2012). Since he was from Philadelphia I asked him if he liked any of the local sports teams. The technique being used is called providing general leads. This technique uses statements or questions that will encourage the client to verbalize (Kozier et. al, 2012).
